The information you provide on this form is considered confidential and will only be shared with the staff and organizers of the HIV and AIDS Care, Treatment and Support Conference in Tanzania for the purpose of reviewing and selecting Scholarship Awards recipients.
Please note that the total number of Awards and Scholarships will be limited. Although the Conference organizers are willing to assist as many people as possible, applicants are strongly advised to also seek sponsorship from other sources. You will be informed of the result of your application. The awards application form can be submitted on paper copy, using the attached form, or downloaded from the conference site and sent by post or courier to:
